Through the Process
Who should I contact?
To apply for Mental Retardation/Developmental Disability
services, or for any questions that arise during the
application process, you will contact the Intake and
Referral Coordinator at the Sedgwick County Developmental
Disability Organization (316) 660-7640.
What are my
responsibilities during the application process?

Keep in regular contact with the
Intake and Referral Coordinator

Schedule and complete the medical exam
with your doctor

Schedule and complete the
psychological exam with a psychologist (for applicants 7
years old and older)

Schedule and complete the
Developmental Delays Checklist with professionals
working with your child (for applicants 6 years old and
younger)

If school aged, obtain and send in a
copy of the most recent IEP

Send in a copy of the individual’s
Social Security Card and Medical Card, if it has not
already been provided.

If found eligible, schedule and
complete BASIS assessment with the Intake and Referral
Coordinator
How long does the
application process take? As the SCDDO does not complete
the testing here, the timeline is truly driven by how
quickly the applicant can get the required testing
completed. Eligibility will be determined within 2 weeks of
SCDDO receiving the completed application packet. The
quicker we receive the testing and necessary paperwork; the
sooner services will be available.
Why do I need a
psychological evaluation? A psychological evaluation is
a comprehensive evaluation needed in order to receive
information about the intellectual functioning along with
any functional limitation the person may experience. The
exam will also give an official diagnosis.
Will my psychological
evaluation work? If you have had a psychological
evaluation completed by a licensed psychologist within the
last three years, it can be considered. Further information
may be necessary.
Why won’t a school
psychologist’s evaluation work? Psychological
evaluations turned into the Sedgwick County Developmental
Disability Organization must be completed by a psychologist
licensed to give a diagnosis. Unfortunately, most of the
psychologists employed through the schools are not licensed
to give a diagnosis.
Will my medical
examination work? Your medical examination will work if
you have had a physical completed by a medical doctor in the
last two years.
Where should I go to get
the DD checklist completed? The Developmental Delays
(DD) checklist is required to be completed for children
birth through the age of six. This form can be filled out by
their medical doctor, teacher, social worker, or other
professional providing services to your child.
What is a BASIS? BASIS
stands for Basic Assessment and Services Information System,
and it was developed by the Kansas Department of Social and
Rehabilitation Services (SRS) to collect information about
the individual applying for the HCBS/MRDD Medicaid waiver.
This assessment consists of three different sections which
will ask questions about: the individual’s demographics,
person’s daily living skills and/or medical conditions and
challenging behaviors, and the kind of services the person
is receiving or is interested in. This assessment is only
completed when an individual is found eligible for MR/DD
services, and is a requirement to receive MR/DD services in
Kansas.
Why do I need to complete
a BASIS yearly? The BASIS assessment is completed yearly
for any person receiving, or waiting for services (unless
they are under the age of five) in their birth month. This
is done to update any changes which may have occurred in the
last year.
What will happen if I have
been determined ineligible for services through the SCDDO?
If you have been found ineligible for services you will
receive a letter in the mail. The letter will give you
information on how you can appeal the decision, along with
giving you other resources in the community to contact for
assistance.
