Eligibility Criteria for Mental Retardation and Developmental Disability Services
Children birth through six:
-
Documentation of at least three developmental delays, AND
-
Reflects a need for multiple services, AND
-
Does not include individuals who are solely severely emotionally disturbed or seriously and persistently mentally ill.
*All eligibility determinations made on children under the age of seven are temporary determinations. Eligibility will be re-determined around the age of seven or eight.
Ages seven and above:
Mental Retardation
-
An Axis II diagnosis of mental retardation prior to the age of 18, AND
-
At least two limitations in the following adaptive skill areas: Communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure, work.
Other Developmental Disability
-
A condition which is evidenced by a severe, chronic developmental disability, AND
-
Is attributable to a mental or physical impairment or a combination of mental and physical impairments, AND
-
Is manifest before the age of 22, AND
-
Is likely to continue indefinitely, AND
-
Results in substantial functional limitations in any three or more of the following areas of life functioning: Self-care, understanding and the use of language, learning and adapting, mobility, self-direction in setting goals and undertaking activities to accomplish those goals, living independently, economic self-sufficiency, AND
-
Reflects a need for multiple services which are identified and managed by a separate person due to cognitive limitations, AND
-
Does not include individuals who are solely severely emotionally disturbed or seriously and persistently mentally ill or have disabilities solely as a result of infirmities of aging.
