What is this service?

Community Support Services (CSS) helps residents with serious mental illnesses learn to live successfully in the community and avoid admission/readmission to a state hospital.

Whom does this program serve?

Consumers of CSS services will have been diagnosed with serious psychiatric conditions such as schizophrenia, bipolar disorder or major depression; may have been hospitalized at least once in their lifetime or have been institutionalized for long periods of time; have difficulty with social relationships, meeting their basic needs; and/or maintaining a home or a job.

How can I get involved in this program?

COMCARE’s Assessment Center at 934 N. Water, provides same-day assessments to new clients and is available when the need for mental health or addiction services arises. Our goal is to connect you with services as quickly as possible. Walk-ins welcome, no appointment necessary Monday through Friday, 8 a.m. to 3:30 p.m.

Services Provided by CSS

Strengths-Based Case Management The purpose of case management in the strengths model is to assist people recover and reclaim their lives by identifying, securing, and sustaining the range of resources needed to live, work, and play as fully integrated citizens of the communities in which they live. It is based on a set of core principles that in part value the unique strengths and attributes of all individuals and views the community as a place of strength and resources. Strengths based case management also holds that the primary setting for our work is in the community.

Community Integration Program - This program involves a variety of psychosocial rehabilitation treatment groups that help consumers manage their mental illness. The primary groups are focused on symptom management, coping skills to prevent crisis, social and recreational skills, pre-vocational skills training, and programs for those with co-occurring substance abuse disorders.

Integrated Dual Disorder Treatment (IDDT) - An intensive approach to treating dual disorders that combines elements of both mental health and addictions treatment into a unified, comprehensive, and continuous program. This model of case management utilizes Motivational Interviewing techniques and Stage Wise Interventions.

Supported Employment Case Management- Employment Specialists provide direct assistance to overcome the symptoms of mental illness related to employment, including a skill and interest assessment, resume writing, interview practice, career exploration, job placement, workplace and classroom accommodations, student financial assistance, coursework selection and other supports related to returning to work or school. Specialists work closely with employers and consumers to enhance job retention.

Peer Support Services - Providers who have their own lived experience with mental illness and mental health treatment provide individual and group supports to help consumers practice recovery skills such as making personal recovery plans, self-advocacy in treatment, goal setting and taking advantage of resources in the community.

Medical Clinic - The clinic staff consist of psychiatrists, advanced practice registered nurses (APRN) and registered nurses (RN) who provide medication and wellness education for symptom management.
