Routine Preventive Screenings

For the past couple of years many of you have participated in the Biometric Screening Process. Last year you were asked to complete both the biometric screen and an online Health Risk Assessment (HRA) to receive the county contribution to your health care flexible spending account. The intent of the screenings was to make you aware of your health and the risk factors that may impact your health in the future. We feel this same outcome can be achieved by establishing a relationship with your doctor. This year Sedgwick County replaced the biometric screenings and health risk assessments with a Routine Preventive Exam with your physician. We encourage you, your spouse and dependents to establish a relationship with your primary care physician and take a proactive approach to your personal health.

The county’s 2015 budget includes a contribution to employees’ flex accounts, the contribution will be made if the employee and spouse (if applicable) visit their primary care physician (PCP) and have a routine preventive exam performed.  The attached verification form will need to be completed by your PCP and returned to Human Resources.  A form is required for both the employee and the spouse (if applicable) to receive the full benefit.  A form is not required for children.  However, we encourage everyone to establish a relationship with your primary care physician and take a proactive approach to you and your family’s health.  We will not request any personal information regarding your health.  Only a signature from your primary care physician confirming you completed an exam.

The verification form is valid for preventive exams performed between October 1, 2013 and September 30, 2014.  Some of you may have recently had a preventive exam.  Simply have your doctor complete the form; past exams are valid as long as they were after September 30, 2013.  If you have any questions, please contact Human Resources at 660-7050 or email benefits@sedgwick.gov.
