Accessing Services

Who can refer me to COMCARE?

You do not need a referral to access COMCARE’s programs and services. We do accept referrals from family members, physicians, hospitals, therapists, the legal system, community agencies, place of employment and other concerned persons.

What to Expect:

COMCARE offers individuals and families several options to complete an intake.  Same Day Assessment is an option available to anyone interested in mental health and addiction services with no appointment required. We will do our best to see you today.

Psychological Evaluations and court ordered mental health assessments are not the same service as an intake for services and will not be provided at the Same Day Assessment.

Please note we are closed on county holidays. Please expect the assessment process to take 45 minutes, up to two hours, once you are seated with a clinician. We suggest filling out paperwork ahead of time to help expedite processes.

What should I bring with me?

Intake Documents

Please complete the documents listed under the Same Day Assessment Program, you intend to utilize. After the documents are submitted, you will receive a call providing additional information to guide you through this process. If you are unable to submit documents online, you may go to any COMCARE location and documents will be provided to you.

Adult Mental Health Treatment Services

To initiate services, please complete the Adult Intake Packet and return electronically.  If you are unable to complete electronically, an intake packet may also be picked up on-site, Monday-Friday, 8am-5pm.

Patients seeking adult mental health services are encouraged to use our Open Access Walk-In days and times below. 

Adult Open Access Intakes are available: 

Monday, Wednesday, and Friday: 8 - 11 a.m. and 12:30 - 2:30 p.m.

Adult Substance Abuse Treatment Services 

To initiate services, please complete the Addiction Treatment Intake packet and return electronically. If you are unable to complete electronically, an intake packet may also be picked up on-site anytime, Monday through Friday, 8 a.m-5 p.m.

Once the completed forms are received, you will be contacted by a staff member to schedule an intake appointment.

Substance Use Open Access Intakes are available:

Tuesday and Thursday: Please arrive by 8:30 a.m. for a morning intake.  For an afternoon intake, please arrive by 12:30 p.m.

Youth Mental Health Treatment Services

To initiate services, please complete the Youth Intake Packet and return electronically.  If you are unable to complete electronically, an intake packet may also be picked up on-site, Monday-Friday, 8 a.m.-5 p.m. 

Patients seeking youth mental health services are encouraged to use our Open Access Walk-In days and times below. 

Youth Open Access intakes are available:

Monday: 8 a.m.-11 a.m.

Wednesday and Friday: 8 a.m.-11 a.m. and 12:30 p.m.-2:30 p.m.

Youth in Foster Care and/or Juvenile Justice System

Documents below are for youth who have been referred by a Foster Care Agency or Juvenile Justice Authority. If the youth is not in the custody of the State of Kansas, you do not need to complete the Universal Packet below, only the youth intake packet.

The Kansas Department of Education Mental Health Intervention Team (MHIT) 

COMCARE partners with five school districts in Sedgwick County. Youth who have been referred by their Mental Health Intervention Team, please select the link below for paperwork and additional information. 


Single Use Forms:

The following documents are included in the program specific Intake Packets above and do not need to be completed again. However, these forms are available for annual updates, when personal information changes or when additions/revisions are needed.
