Process to Request a Lost Vaccination Card
Release of lost CDC COVID-19 Vaccination Cards and Electronic COVID-19 Records administered by Sedgwick County Health Department at the Main Vaccination Clinic or Mobile Clinics:
If a patient needs a new CDC Vaccination card due to being lost and the patient was vaccinated at a Sedgwick County Health Department Clinic or Mobile Clinic, the patient can request a new card via:
- Email medrecordsrequest@sedgwick.gov (will require ID and Release of Information form)
- Walk-in to the Health Department Clinic at 2716 W Central (show of ID required) May require a wait.
- Call COVID-19 Medical Records at 316-660-7300
If the patient needs a copy of their immunization records showing the COVID-19 Vaccination record for domestic or international travel and the patient was vaccinated at a Sedgwick County Health Department Clinic or Mobile Clinic, the patient can:
- Email the medical records department at medrecordsrequest@sedgwick.gov (will require an ID and Release of Information form to be completed)
- Walk-in to the main Health Department Clinic at 2716 W Central and have a Fiscal Associate print off a copy of the Immunization record. (Show of ID required) May require a wait.
- Call COVID-19 Medical Records at 316-660-7300
If the patient needs a copy of their immunization records showing the COVID-19 Vaccination record “verified/stamped” to submit for international travel and the patient was vaccinated at a Sedgwick County Health Department Clinic or Mobile, the patient will need to:
- Schedule an appointment for “Immunization Medical Counseling” with an immunization nurse at the Sedgwick County Health Department Clinic at 2716 W Central.
- To schedule an appointment call 316-660-1100.
NOTE: Sedgwick County Health Department cannot release or complete new CDC vaccination cards for patients that did not receive their COVID-19 Vaccines with us. The CDC replacement cards will need to be provided by the facility that vaccinated you.
