Process to Request a Lost Vaccination Card

Release of lost CDC COVID-19 Vaccination Cards and Electronic COVID-19 Records administered by Sedgwick County Health Department at the Main Vaccination Clinic or Mobile Clinics:

If a patient needs a new CDC Vaccination card due to being lost and the patient was vaccinated at a Sedgwick County Health Department Clinic or Mobile Clinic, the patient can request a new card via:

• Email medrecordsrequest@sedgwick.gov (will require ID and Release of Information form)
• Walk-in to the Health Department Clinic at 2716 W Central (show of ID required) May require a wait.
• Call COVID-19 Medical Records at 316-660-7300

If the patient needs a copy of their immunization records showing the COVID-19 Vaccination record for domestic or international travel and the patient was vaccinated at a Sedgwick County Health Department Clinic or Mobile Clinic, the patient can:

• Email the medical records department at medrecordsrequest@sedgwick.gov (will require an ID and Release of Information form to be completed)
• Walk-in to the main Health Department Clinic at 2716 W Central and have a Fiscal Associate print off a copy of the Immunization record. (Show of ID required) May require a wait.
• Call COVID-19 Medical Records at 316-660-7300

If the patient needs a copy of their immunization records showing the COVID-19 Vaccination record “verified/stamped” to submit for international travel and the patient was vaccinated at a Sedgwick County Health Department Clinic or Mobile, the patient will need to:

• Schedule an appointment for “Immunization Medical Counseling” with an immunization nurse at the Sedgwick County Health Department Clinic at 2716 W Central.
