I understand that any incorrect information submitted on this application may result in the denial of this permit and the forfeiture of the application fee. The permit will only be issued after an on-site inspection by a representative the Sedgwick County Fire Prevention Office. This permit is not transferable to another applicant or site and it shall be visually posted at the site and surrendered upon request to the representative of the Sedgwick County Fire District 1. I further understand that no one is permitted to discharge any type or form of firework within 100' of the storage container. (required)
