Employee Relations Form

Sedgwick County is dedicated to serving all employees and providing a safe and confidential reporting tool for personnel issues. This form may be used to report a violation of policy specific to personnel matters. The subject matter relayed in this form will be reviewed by Human Resources. You will be contacted by a Human Resources staff member to follow up on your concern and address next steps. The information provided will not be shared outside of Human Resources without your permission. Exceptions to this are reports that involve immediate safety concerns.
