Passport Photographs
- Passport photos can be taken in the office for a $15 fee. Two passport photos will be provided.
Photo requirements:
• Must be 2 x 2 inches
• Must be in color
• Must be a clear image
• Must have a plain white or light-colored background
• Head measurement must be between 1 inch and 1 3/8 inches from chin to top of head
First-Time Applicants (Form DS-11)
Complete Form DS-11 in black ink
• Be a U.S. citizen
• Appear in person
• Provide proof of citizenship
• Provide proof of identity
• Bring required payment
Applicants under age 16:
• Both parents or legal guardians must appear in person
• If both parents cannot be present, additional documentation is required
Applicants age 16–17:
• Applicant must personally sign the application
• One parent or legal guardian must be present, or the applicant must provide a letter of consent from one parent or legal guardian
Proof of Citizenship

You must provide one of the following:
• Certified U.S. Birth Certificate with raised seal
• Naturalization Certificate
• Report of Birth AbroadHospital birth certificates are not accepted.
Proof of Identity

You must provide a valid government-issued photo ID such as:
• Driver’s License
• State ID card
• Military IDFor children under age 16, a parent or guardian ID is required.

Passport Renewal (Form DS-82)
You are responsible for mailing Form DS-82 to the National Passport Processing Center.

Routine service: 10 to 12 weeks
Expedited service: 4 to 6 weeks
