WHAT YOU NEED TO KNOW:
- Appointment REQUIRED (No walk-ins)
- Parent/Guardian must be present with child.
- No out-of-pocket cost.
- No cost for students who do not have insurance
- No cost for students who have Medicaid
- No co-pay for students who have private insurance
WHAT YOU NEED TO BRING:
- Vaccination records
- Health Insurance Card
(if applicable) - Required Immunization Letter from School Nurse (if applicable)
