By submitting this application, I am stating that I am the owner and full time occupant, and I am taking full responsibility for work done under authority of this permit.

I agree to follow the inspection procedures for this permit as required by MABCD. I understand that I will be responsible for providing any required engineering, documentation, and/or to make corrections as required by the Building Official or his authorized agents.
